Needs, Risks, Safeguarding concerns,Strengths, Wishes and Preferences are recorded during Assessments as Observationsin referenceto the Subject of Care and can form a part of the Assessment record, Care Plan record or Observations record. Assessment are performed for the Subject of Care and can be expressed by a Proxy for the Subject of Care if the Subject of Care does not have capacity. Needs Assessments evaluate the Needs, Wishes, Strengths and Preferences care. These Observations about the Subject of care influence both assessment and care planning processes and are recorded before, during and after Assessments.

Care Needs Assessments evaluate the Needs,Strengths, Wishes, Preferences of the Subject of care. Needs can include information about the significanceand urgency of the care need as well as and/or associations with a list of healthcare problems and further detail e.g. stage of the disease etc. Wishes, Strengths and Preferences . Strengths, Wishes, Preferences are evaluated and recorded as a narrative description.

Risk Assessments Assessments identify Risks and Safeguarding Concernsconcerns, as well as recording why thereason for the risk assessment was conducted, who , the other Care Actors the risks involves and if theSubject of Care understands carehas an understanding of the risks. Risks can include information about trigger factors, relapse indicators and information about the Care Actors who should be informed of the risk.

Mental Capacity Assessments, include includes a narrative of whether an assessment of the mental capacity of the (adult) person Subject of care has been undertaken, if so, . The narrative statement includes what capacity the decision relates to and , the outcome of the assessment . Also record best interests decision and the best interest decisions if person lacks capacity, as well as . Mental Capacity Assessmentrecords should include a reference to the Document and DoLS record if relevantlocation of the Document containing the mental capacity assessment information.

Assessmentrecords should include details about a scheduled Review.
